Navigating your child's health journey can be complex. We've gathered our most frequent inquiries so you can find clarity quickly. Can't find your answer? Contact us directly →

Please arrive at least 10 minutes early to complete any necessary paperwork or update your child's insurance information. If you arrive more than 15 minutes past your scheduled time, we may need to reschedule or see your child as a work-in depending on availability. We do not accept walk-ins—please call our office at (678) 820-7979 or schedule online through the patient portal.
We complete all necessary forms for schools, daycares, and sports organizations—free of charge when done during your child's annual wellness exam. Sports physicals and standard forms (3300 and 3231) are included at no cost at the wellness visit. Please bring the forms with your portion already filled out. If a standalone sports physical is needed outside of a wellness visit, a fee applies and completion may take up to one week. Note that each organization uses specific forms—confirm the correct form with your child's coach or school before your appointment.
For the initial visit, a parent or legal guardian must be present. For follow-up visits, another adult may bring your child provided you have given written permission authorizing them to accompany the child and make medical decisions during that visit. That adult must present a valid photo ID at each visit. You must also be reachable by phone in case of emergency. The same policy applies for teen patients ages 16–17 wishing to attend independently. Patients 18 and older may come to appointments without parental consent.
Braselton Pediatrics respects every parent's right to make informed decisions about their child's care, including vaccination. We do accommodate alternative and delayed schedules with the understanding that families sign our non-vaccinating agreement and maintain all scheduled well-child visits. Please contact our office to discuss your preferences and obtain the agreement form. We are always happy to answer questions and provide education on each vaccine.

Braselton Pediatrics respects a parent or guardian’s right to make informed healthcare decisions for their child. Families who choose not to vaccinate or who elect to follow an alternative vaccine schedule must agree to the following practice policies:

  • At each well-child visit, our providers will review the vaccines currently recommended for your child and discuss their benefits, risks, and timing. Vaccine information statements and other educational materials may also be provided.
  • A vaccine refusal acknowledgment must be reviewed and signed at each well-child visit when recommended vaccines are declined.
  • Braselton Pediatrics follows the immunization schedule recommended by the American Academy of Pediatrics.
  • Patients who are unvaccinated or following an alternative vaccine schedule must remain current with all recommended well-child visits.
  • Recommended well-child visits generally occur during the newborn period; within the first week of life; at 1, 2, 4, 6, 9, 12, 15, 18, 24, and 30 months; at age 3; and annually thereafter.
  • Failure to maintain the required well-child visit schedule may result in dismissal from the practice. We understand that emergencies and unexpected circumstances may occur, and these situations will be reviewed individually.
  • Families are responsible for meeting all immunization and documentation requirements established by their child’s school or childcare facility. When a family decides to begin or resume vaccination, our providers will help develop an appropriate catch-up schedule. Some vaccines have age-related recommendations or limitations.
  • Braselton Pediatrics does not prepare vaccine exemption or waiver letters.
  • Georgia Certificate of Immunization Form 3231 can be completed only when the patient’s immunization record meets the applicable requirements for issuance.

Please contact our office at (678) 820-7979 if you have questions about this policy or would like to discuss your child’s immunization plan with a provider.

This allows us to check for jaundice, monitor weight gain, and support you with any initial feeding or newborn care questions. We are here to help new parents feel confident and supported from day one—please don't hesitate to call with any concerns.
We accept most major PPO insurance plans. Please contact our office directly at (678) 820-7979 or have your insurance provider verify in-network coverage prior to your visit. For patients without insurance or who prefer self-pay arrangements, we offer transparent pricing—please contact our billing department for details.
We understand that illness does not follow business hours. If your child needs urgent guidance after our office closes, please call our main number at (678) 820-7979.
